The COVID-19 pandemic has had an immense toll on health care, the global economy and our everyday interactions. The lack of effective treatment for severe cases of disease has played a huge role in the growing number of infections and deaths worldwide, which continue to rise each day. This situation has motivated several stakeholders to promote novel strategies for the reduction of transmission of the disease based on the limited evidence currently at our disposal. Such is the case of the use of mouthrinses whose apparent in vitro effects on the viral load of SARS-CoV-2 have led to their being proposed by certain organizations as a potential intervention to reduce transmission of the virus by lowering its concentration in the saliva of infected individuals. Were this to be truly effective in clinical trials it would prove to be a valuable tool in our fight to contain the spread of disease. For this reason, our objective was to conduct a living systematic review of human clinical trials to assess the impact of mouthrinses on the viral load of SARS-Cov-2 in the saliva of infected individuals. Methods. This study was reported using the PRISMA guidelines. An electronic search was conducted in seven databases and preprint repositories. We included human clinical trials that evaluated the effect of mouthrinses with antiseptic substances on the viral load of SARS-CoV-2 in the saliva of children or adults, who tested positive for SARS-CoV-2 by reverse transcriptase polymerase chain reaction (RT-PCR). The risk of bias was assessed using the ROBINS-I tool. PROSPERO registration number: CRD42021240561.
